Texas Advance Directive Act
    The full text of the act, as updated in 2003, is provided at this link.  It is rather lengthy, but is well worth reviewing.
Texas Sample Advance Directive
    While one may use any form one wants in Texas, this is the sample directive put forth in the Act.  Use of a directive with this form provides certain addition benefits and guarantees under the current law.
Medical Power of Attorney
    This is the MPOA form that the Act puts forth as an example.  It includes an explanation of how to fill out the form.
Texas Out-of-Hospital Do-Not-Resuscitate (OOH-DNR)
    Emergency Medical Responders must institute life-prolonging treatments unless they are presented with this form.  The Texas Department of Health also is very particular about the way this form is completed.  For example, the original form must be front and back on one sheet of paper.  The directions for completion are on the back of the form.
Physician Requests Withdrawal of Treatment Information
   The TADA provides for a due process mechanism for dealing with situations when the treating physician feel a treatment or procedure is futile and the patient or family wish to continue treatment.  This is the TADA mandated language to explain this mechanism.
